• Helping clarify medical information in real time
• Taking notes and identifying follow-up questions

Medical Appointment Support (In Person)

$400 per appointment

Includes:
• Attending the appointment with the patient
• Helping ensure concerns and questions are addressed
• Clarifying diagnoses, testing, and treatment plans
• Brief follow-up summary after the visit

Post-Appointment Review / Follow-Up Consultation (30 minutes)

$175

Includes:
• Review of information discussed during the visit
• Clarification of medical information discussed by the healthcare clinician
• Assistance identifying the planned next steps or questions still to be answered

Health Record Review / Care Navigation

$250 per hour

Includes:
• Review and organization of medical records to assist with communication with the care team
• Assistance identifying appropriate specialists as needed
• Guidance navigating complex healthcare systems

Services are private pay and not billed to insurance.

Payment due prior to time of service via secure online payment or check by arrangement.

Designed for patients who have an important upcoming appointment.

Includes:
• 60-minute consultation (review of history, concerns, goals)
• Help organizing medical information and key questions
• Written list of questions for the upcoming appointment
• Brief follow-up summary

• New diagnoses
• Specialist consultations
• Complex test results

2️⃣ Appointment Support Package

$650

For patients who want support before and during a medical visit.

Includes:
• 60-minute preparation consultation
• Help developing questions and priorities
• Participation in the appointment (video)
• 30-minute follow-up review afterward

• Cancer consultations
• Major treatment decisions
• Complex care planning

3️⃣ Comprehensive Navigation Package

$1,250

For patients navigating complex or serious medical conditions.

Includes:
• Initial 60-minute consultation
• Health record review for organization and communication with care team
• Assistance identifying appropriate specialists or resources
• Participation in one appointment (video or in-person)

•     One 30-minute follow-up consultation with your patient advocate

• Up to 2 additional hours of navigation support for care coordination, questions, and guidance, to be used within 60 days of enrollment

• Rare or complex conditions
• Patients feeling overwhelmed by the healthcare system

Optional Add-On Services

• Additional appointment support (video) – $325
• Additional appointment support (in person) – $400

Packages provide bundled support for patients navigating complex medical care.

If preferred, can bill as hourly rate $275/hour to include any of the above services. Requires 5 hour retainer to begin services.
